This study examines how the implementation of the Kurt Lewin Change Management Model can improve organizational performance in health service settings. The main problem explored is the slow adaptation of health workers to new work patterns, which often reduces service quality. The purpose of this study is to analyze the effectiveness of the unfreezing, change, and refreezing stages in supporting behavioral adjustments among employees. Using a descriptive qualitative method, data were collected through interviews, observations, and document analysis within a primary healthcare clinic. The results show that proper preparation, clear communication, and continuous reinforcement significantly increase staff readiness and service efficiency. The conclusion indicates that the Kurt Lewin model is useful in guiding structured organizational change and strengthening performance improvements in health service institutions.
